Reasonable Construction of Medical Ethics Principles: Based on Critique of Principlism

摘要: 中国的医学伦理学亟待建构一套符合自身需要的伦理原则体系。自20世纪90年代开始，比彻姆和邱卓思的原则主义逐步为我国学术界所接受，但这一理论体系却存在着诸多的缺点。从医疗情境的视角来看，原则主义不仅在伦理原则的析出方式上忽视了医学道德的特殊性，还在伦理原则的应用上存在着价值标准的模糊性。建构中国的医学伦理原则体系，要充分尊重医学实践的规律，并且认真考量当代中国医学事业的实际情况，以医学目的的实现作为价值导向，同时注意解决好伦理原则如何有效指导实践的问题。Abstract: Chinese medical ethics needs to construct an ethical principle system that meets its own specific needs. Since the 1990s, the principlism of Beauchamp and Childress has been accepted by Chinese academic community. However, this theoretical framework has many shortcomings. From the perspective of the medical context, principlism not only neglects the particularity of medical ethics in the way ethical principles are derived, but also shows ambiguity in value standards when applying these principles. To construct a system of Chinese medical ethics principles, it is necessary to respect the laws of medical practice and to consider the actual situation of contemporary Chinese medical development and take the achievement of medical goals as a value orientation, while also addressing how ethical principles can effectively guide practice.
